Unfortunately no - The units have been installed at the clients factory.
D0 - Serial OUT
DI - Serial IN
V - 3.3V in
G - Ground

This should be enough - the datasheet for the level shifter : http://www.proto-pic.com/Datasheets/4050/4050.pdf

you have the wrong board selected 'Arduino Duemilanove or Diecimila, ATmega328'

Yes I've used these & used jumper wires (Male to Female) on a 3.3V Arduino, and through a level shifter (4050) on a 5V Arduino.

take all the wires out
redo them
if all is as you described, it should work
double check the power and ground with your multimeter (some breadboards have split rails)
