What is the purpose of connecting a yellow wire on dc motor from the chip ?

Probably the part number and the date code.

You will have to provide a much better photograph and description. It could be the positional feedback signal if it is a servo but there is not enough context to answer this question.

The yellow wire appears to be connected to the motor case so it would likely be for shielding or electrical filtering.  If you don't care about electrical noise you can probably leave it disconnected.
