If you look at the Arduino Uno schematic
you can see the connections between the 6-pin header (labelled ICSP) and the atmega328 (labelled ZIC1). Use the same connections on your own hardware. Yes, RST goes to the Reset pin, and you also need a 10K pullup resistor between Reset and Vcc. VTG goes to +5v.
