arduino UNO Input Voltage (limits)   is 6-20V, what happen if I give 5V?

Through the Vin or jack barrel then it would not get enough voltage from the regulator to work correctly. Through the 5v pin however it would be fine.

I did tried it, and it work fine for a month. Then it fail to function. So what went wrong, the board overheat because of low voltage?

Yes but then it should not have functioned in the first place.
Does it work if you plug it into the USB.
The other thing it could be that you applied inputs greater than 0.5V above the power pin, as that was probably only at 3.5V a 5V input could kill the board.

No not directly, something else did that might not have killed the board if you had powered it properly.

So using a 5V power adapter made the board more vulnerable to other things.

Did you measure the output voltage to verify it is regulated at 5v? My arduino has had voltage excursions above 5v and survived.

Zoomkat you are missing the point this 5V was supplied through the regulator and so produced an output voltage of less than 5V. Excursions over 5V would have been helpful.
