is it possible to write a form with 6 input boxes and a send button so that the values inserted can be sent to 6 different pins on the arduino  ?

Yes, it is. The form can have 6 input boxes, as your post suggests, or 3 as your topic suggests. What happens when you press the send button is what is critical.

That, and having the Arduino collect and parse all the data sent, with appropriate error handling to deal with dropped data.

Possibly yes, but you really define what you are talking about when you say "form". Is it a printed pdf form filled out with a pen, some MS word template, a web based form, or something else?
