MuRata overvoltage problem?

Has anyone heard about the MuRata module failing due to being connected directly to the USB 5 volts (VDD_USB)? The MuRata data sheet shows pin 4 connected to 5 volts, but the spec sheet says pin 4 cannot be over 3.6 volts.

It looks like Arduino is aware of this issue:

They are planning to make an r2 of the board with fixes for that and another issue that causes excessive power usage:

Yeah, same question here. Actual device shows ABX00017-B, but what was updated?
