I am trying to use a LTV-844 as an isolator for a 120vac to 24vdc circuit. The LEDs are just for the example.
The issue I am having is I am getting 120vac across all inputs when only one is on.
What am I missing?
The circuit looks okay with respect to the problem you cite. So maybe your implementation doesn't follow it exactly...

As @anon57585045 says, the schematic looks OK.
Have you checked that the PLC is actually only activating one output?
Can you post some good, clear photos of your implementation?
Does the PLC have a snubber circuit connected across its contacts that will allow a small current to flow through your opto-isolator?
