Why don't you contact the owner of the Flickr account that posted it and ask him? His description said he designed it, so he should know if and where it's available.
You can find these boards from all sorts of Chinese vendors. Some examples:

As a followup, we just assembled our first relay card product. First time doing double sided SMD also.
It plugs onto a Mega, has 5V coil SPST relay with 5A 250V/30VDC contacts with screw terminals for connecting in the hot lead of whatever is being controlled.
Once I find the PL behind it, I can post a price.
MrsCrossRoads is also asking if she can hook up Christmas lights with it.
Has an LED to indicate when each relay is on, and A22-A29 drive NPN transistors on the bottom of the board to sink coil current.
Coils can be powered from 5V, Vin, or from an offboard source via its own barrel jack (not installed here) or via 2 screw terminals.
I have an Uno version ready, but ordered the wrong shift register, so we haven't assembled it yet.
Go to ebay and type "arduino 8 relay" into the search. I think you'll find they're pretty pervasive.
All are standalone modules tho (I did see one 6-relay Uno shield with all thru-hole parts, looks very clunky in comparison). I am going for something more integrated.
I'd say that CrossRoads' design is far more durable around the terminals section - except for that terrible blunder with "fill"!
Come on!
You need to fix that before marketing!
