I think you're right:
It seems that this is intentionally not customizable with arbitrary text to avoid people using it for mischief. I think what happened is that anyone who had a custom title in SMF (or whatever the equivalent field was called in that software) had that migrated. But they won't ever be able to change that text (though they can choose from the other fixed titles on the list in that menu fratt mentioned.
Note this tip in the thread I linked above though:

not sure i see the value of the karma titles.
seems like a useless features when other features need improvement
