FTDI also needs DTR connected to Reset via an 0.1uF cap.
Or press Reset on the Lilypad when the IDE shows "compiled xxx of 32xxx bytes" to start up the Lilypad bootloader code.
Can be easier to get the timing right if you select File:Preferences and turn on Verbose outputs.
