Which USB-TTL adapter are you using?

FTD1232

I suppose you've already checked and re-checked your settings, but just in case:

Are you sure you have the right Card/Processor/Speed/Programmer selected in the Tools menu?

I got caught out when I changed between ATmega 328P 16MHz and ATmega 328P 8MHz.

Tools/Type of card/Arduino Pro or Pro Mini
Tools/Processor/ATmega 328P (5V,16MHz) (or whichever one you have)
Tools/Programmer/AVRISP mkII

Is your FTDI voltage jumper in the right place?
