What do you mean by "notifications"?
If you mean the forum's native notifications, which are shown when you click your profile picture at the top right corner of the forum web page, then in my experience they do work reliably.
If you mean the "live notifications" that are sent via the web browser application, I don't know because I don't use them. The reliability of these will be dependent on the reliability of the web browser application in addition to the forum software.
If you mean email notifications, I did use them at one point and found them to be reliable, but with a caveat: the forum software will send you a maximum of 100 email notifications per day. So if you are watching so many categories and/or topics that you would receive more than 100 notifications in a day, then you will not receive notifications for that additional activity. Although this is the system working as designed, you could consider it to result in "unreliability" if you are relying on the forum software to send you an email for all activity, even if that would exceed 100 emails per day.

Yes, and I believe I have them turned off, whcih is what I want.
But what is the default setting?
New members may be expecting notifications, but they aren't set.

Again, you didn't specify what exactly you mean by "notifications", so it is not possible to provide a concise answer to your question.
However, I do think you have a valid concern. I remember that when I joined the forum in 2013 (note that this was long before the switch to the Discourse forum framework) I was quite frustrated to discover only by chance while browsing the forum that I had received replies to my topic. I had expected that I would receive an email notification if anyone replied, but found that they were disabled by default. I always had a hypothesis that this default was one of the causes of the frequent problem we have here of people making a topic requesting assistance, but then never replying on the topic when the forum helpers request additional information.
Here are the current default settings, which new users will have until such time as they adjust them:

So it looks to me that, unlike how it was in 2013, the current defaults are quite reasonable and should indeed result in new users being notified of replies in their topics.
