Put large pads for 4 & 5 and connect them to the ground plane on the PCB. The pads should be large so that they can take more solder and make a mechanically strong bond between the socket and the PCB. For your other questions, it depends on how the plug is wired. Can't select multiple solutions.

Yeah forward and backwards for the solenoid by the SSR's (12v) and then the motor off of Arduino +5v
