Controlling program via web

So I'm looking for some help with a project. I have a program written to control a 4 channel relay shield to turn two peristaltic pumps on and off at specific times for a specific amount of time. Ideally I'd like to be able to control these variable via a simple web page. The problem is, I'm a complete idiot when it comes to figuring this stuff out. I'm wondering if anyone would be able help me port this code over to a web accessible control page.
