I uploaded the Sketch and the monitor showed:
Starting w5100
192.168.2.2
I went ahead and removed this section after I tested your code:
// disable SD card if one in the slot
pinMode(4,OUTPUT);
digitalWrite(4,HIGH);
And the serial monitor showed the same information.
So...what does this indicate? An issue with my router perhaps?
