php is a scripting language, not a web server. Apache is a web server.
A server provides data. A client connects to the server to get the data. When it connects, it specifies some script on the host machine to be executed. That can be a php script.
What, exactly, is this php script doing to define the relay/LED state?

Zoomkat, I tried your sketch, but whenever I clicked "Off" the relay turned off then turned back on. Any idea why it does this?
Perhaps you are trying to power your relay directly from the arduino? If it is, that might cause the arduino to reset. Disconnect your electronics from pin 5 and check with a multimeter the power switching. Note that if you use the browser back button, the previous page with its selection may be reloaded.
