HC-SR04 Ultrasonic range finder is not working

Hi, I am using a HC-SR04 Ultrasonic range finder to measure distance using ultrasound. However, the sensor is not measuring any distance and the reading on the serial monitor is “Out of range”. Also, the red LED is always on.
