ebay is full of them, most coming cheaply from China.
There are 3 sizes in SMD: 603 (smallest), 805 and 1206 (biggest).

More than that (unfortunately.) PLCC2 and PLCC4 are the common types you left out (plus there are some 0402 TINY ones.) Plus SMT versions of leaded parts (mostly bigger.) Plus SMT high-power LEDs. Sigh.

I've been pretty happy with some PLCC2 LEDs from http://stores.shop.ebay.com/Experimenters-Discount-Warehouse__W0QQ_armrsZ1
