How do you name your sketches?

I wonder how others name or organize there sketches.

I cave them names like:
RFM12_over_Serial_Mega
RFM12_over_Serial_Nano
DS18B20_Nano_RFM12B

But after doing other stuff for 3 days I can't remember what is what and what
code i uploaded to which Arduino.
Also i first try to make one thing after the other getting to work and then combine them
into one sketch.
