How did you calculate IR_SAFE (220ohm).
Did you ever look at VCC of that chip with a scope.
R value seems very high for a power IC, and C3 seems to low.
Which Arduino are you using.
With a 1:5 divider for voltage I expect you're using 2.56volt Aref?
