Really a transformer or a PSU? Where is it connected to? Gnd and Vcc do not look like motor power connectors. Have a data sheet for the driver module?
Please supply the data shield for your motor. A DC motor has 2 wires, what's the remaining wires and are they connected somehow to the motor?
