You definitely need an external antenna, as there is no antenna installed on the board and the modem will simply be destroyed without the use of an external antenna.
The fun fact is that I only know of the Arduino store, which with the MKR boards also "gives" an external antenna. Mouser, digi-key, german Reichelt etc. deliver without an external antenna.
