Under the tools menu is the option to choose a programmer like "AVR ISP" etc.
I can't turn up any reference to the use of the various programmers.
How do I decide which to use for a specific board ??

You buy a programmer, then, when you burn the bootloader (or your own hex file) you select it from the list.
You do not need to have a programmer, nor do you need to set the "programmer" value in the menu, for normal user-level use of an Arduino (writing and uploading sketches.) It would be used for adding the bootloader to bare chips, or for other "fringe" applications of the Arduino HW.
