Rev2 M4 output failure

Just getting started on my Rev2 and connecting the self-balancing motorcycle motors. I'm not seeing any output from the M4 outputs on the board. In summary, both motors work with M3, both fail with M4. Both motors also partially work with M1 and M2, but they never turn off after a Simulink run, I have to pull the USB cable and power down the board. Looks like a bad board.
