Has any body used Microsoft Visual Studio to develop the web pages to go on the Arduino Web Server SD?

Should work fine. Stay away from any server side processing like ASP.

Anybody done it cos I am not sure which files to place on the SD from VS.

You just put the developed html file on the SD card, then serve it to a client when requested.
