The Vcc and AVcc pins should both have a ceramic 100nF capacitor near the pins going to GND.

Just because you got away with it once doesn't mean that it won't ever bite you!
In fact, this is exactly the sort of thing that can cause a thing to work "most" of the time - and then sometimes not work for no obvious reason ...
When I recently built my 328PB breakout board I noticed that the 22pf resonance capacitors are the maximum value of the recommended 12-22pf range. But the pin capacitance is also 6pf, so in fact 22pf may be too high.
I'd like to do some testing with lower value capacitors (e.g. 10pf) to see if that gives me a more resilient resonance.
Tried everything, but still no sucess. Anyone have more tips?
