Where can I buy mounting plates for nominal 1/2" threaded-cylindrical-body toggle switches (with the anti-rotation tooth in the hole)?
How would anyone know where in the "plate" to put a hole and what size of plate you need. People usually just drill a hole in their plate where they want it.

The "key" that fits the slot milled out of the threads is separate peice, like this?

It isn't a separate faceplate with a square tooth left in the otherwise round hole?
DIY, use a step drill to make the hole.
Use a file to make the slot.
An ON/OFF toggle switch shouldn’t really need the slot to stop switch turning.
They usually come with a tab that fits a 1/8 inch hole to lock the switch in place. Is this what you have?
Try searching for 15/32" keyed chassis punch.
I’ve use a great many of those switches over the years.
They only need a round hole for mounting as there is no turning action; just a back and forth toggle action happens.
If you must, use a shake proof washer behind the panel.

Do you see the two nuts on the switch? The knurled nut will be flush against the panel front surface. The hex nut goes behind the panel and is tightened against the back of the panel. final tightening of the knurled nut and you are done.
If you have a wrench that can tighten the back switch, use it to fix the switch in place.
You want to end up with none of the switch barrel beyond the knurled nut.
A hunk of hot glue could be used to keep the switch from turning but from experience, your switch will not turn.

This piece would go over the switch threads between the inside nut and the rear of
the panel. A hole in the panel is drilled and positioned to accept the bent tab.
