Windows 10 - ATMega328p - Upload Error

Hi. I made my own arduino from the atmega328p microcontroller. I used a CP2012 USB-TTL converter to upload the code. I tried to upload and I got this error message after a 4-5 minutes:

Can someone help me ? What can i do ?

Yes. I checked the wires 5-6 times, and the controller has a bootloader. But i tried to replace my controller in the arduino uno and upload a programme. And it’s Works. So i dont know what is the problem…
