I am using an SD card adapter for my Arduino Mega but everytime I try to run the CardInfo example all I get is this message.

I've formatted the SD card, I've checked the wiring, I've checked if it's going to the right pins, I've tried multiple cards and adapters. I don't understand why it can't detect the card.
Which SD card adapter are you using?
How is it connected to the Mega?
Some use D4 as CS. Others use D10.
You are aware that the SPI pins on a Mega are not on pins 11/12/13?
For some reason this has happened to me, too. An easy solution would be using SdFat.h. It has more functionality compared to the default SD.h, including the option to format the SD card using the Arduino board. Since I've used it, this never happened...

Yes, I've used an Arduino Mega 2560 (clone, actually), but instead of an SD card, this happened to my MicroSD card...
