Are there any notations on the encoder, like manufacturer or part number?

I can't find a data sheet for the encoder so you will have to figure the pins out your self. Do you have a DMM (Digital MultiMeter)?
A search for "FIND PINOUT OF OPTICAL ENCODER" yields some pages with information.
