You'll need some app running on the PC to do that, try Python maybe?
Have a look at these tutorials, especially 10 and 11.
You don't have to use a PC, you can draw live graphs direct to LCD on Arduino and, while using Python might be a nice intellectual exercise, it is really just reinventing the wheel.
Using the Henning Karlsen's library is quite easy.

The actual draw commands are quite straightforward.
You can do it on a PC using PLX-DAQ. This is a macro for older versions of Excel. It essentially turns Excel into a terminal and enables you to use all of Excel's graph ability in real time. You don't need to send any time data, as Excel gets that from the PC clock. You also get an XLS file at no extra charge.
The live graphs I do most are on the phone using Bluetooth Graphics Terminal. This app is about the best reason for getting a smartphone you will ever see.
In both of the above methods you make just minor alterations to the serial data you are already using.
If you really want to impress the natives, you can sand the data to Processing

The first live graph I did was to send the data to cosm (now xively) and pick it up on the PC. It sounds a bit extreme, but they get to see it in Vladivostok too, and it beats the hell out of using Python.
Two other painless methods I have not used are:

and I'm sure there are plenty of others.
